Most likely causes:

This is usually caused by a problem / bug in the agent.
Each SNMP tables row is identified by a set of indexes. These index values are required to be unique across the table. For table ${table.name}, the agent returned more than one row with index value ${index.value}.
If not detected, this problem would cause in infinite loop when trying to 'walk' the device.
The most likely solution would be to identify the bug causing this issue and to upgrade to a firmware version where this bug is solved.
